    RBMA’s Payor Relations Committee (PRC) wants to alert RBMA members to a potential issue that could negatively impact digital mammography payment in 2015.

    In the proposed rule for the 2015 Medicare Physician Fee Schedule (MPFS), the Centers for Medicare & Medicaid Services (CMS) proposed to delete the existing HCPCS codes for digital mammography (G0202, G0204 and G206) and reassign their relative values to the analog CPT® codes (77057, 77056 and 77055, respectively) in view of the prevalence of digital mammography. 

    In the event that this proposal becomes final, the PRC recommends that members review all payor contracts soon to assess the potential payment impact of deleting G0202-G0206 and using 77055-77057 for all mammography, especially if you perform global or technical component (TC) digital exams.

    In general, if contractual payments are based on current year MPFS rates or RVUs, this change likely will be close to neutral for the professional component (PC; -26) regardless of the type of mammographic technology (digital or analog).  For global and technical component (TC) analog mammography, the change could potentially be positive since the TC RVUs for digital mammography are significantly higher than those for analog mammography. However, each contract must be reviewed to ensure updates to MPFS rates or RVUs occur on January 1, 2015. 

    If you have contract rates for global or TC digital mammography determined on some basis other than current year MPFS rates or RVUs, there is the potential for a significant negative impact.  For example, if your contract specifies higher payments for G0202 than for 77057, your 2015 digital mammograms (reported using CPT® codes) would likely be paid at the 77057 rate unless you take action. You may want to: (1) educate your payors about this issue, (2) recommend that they follow CMS’ lead with respect to transferring of rates/RVUs from G0202-G0206 to 77055-77057, and/or (3) renegotiate such payor agreements.

    Issues related to Medicaid fees and Tricare fees may exist as well.  Some of these fee schedules do not update on January 1st of each year and can pose a potential risk to your payments from these agencies.

    RBMA members should begin reviewing their contract provisions related to mammography and not wait until CMS’ final rule comes out in early November because there may not be adequate time to address the issue with payors prior to January 1, 2015.

    On a related note, members should be aware of the potential for a new CPT® code(s) for breast tomosynthesis in 2015.  This issue may present an opportunity for a related conversation with private insurers about payment for breast tomosynthesis.
